It is when a medium is able to feel pains that may be associated with how someone in spirit passed away, or to detect pains in ta client when doing a healing for example. A spirit guide is a spirit person who helps a medium to do their work from the world of spirit. Some spirit guides stay with you all your life; others come and go as you evolve, learn and grow.
